Many physicians believe that timely notification of critical laboratory results can pivotally affect patient outcome. (1) Sometimes referred to as critical values or panic values, critical results generally comprise less than 2% of all laboratory results. Among surveyed physicians, 95% consider calls about critical results to be helpful in management, 75% note critical results in the medical record, and two-thirds of critical results prompt some change in therapy. (2) A number of regulatory and accrediting bodies require the reporting of critical results on a timely basis. The Clinical Laboratory Improvement Amendments of 1988 (CLIA) require that critical results be reported "immediately." (3) The College of American Pathologists (CAP) has identified reporting of critical results as a National Laboratory Patient Safety Goal. (4) The Joint Commission (JC) requires that all critical results be reported to "the responsible, licensed caregiver" within time frames established by the laboratory (defined in cooperation with nursing and medical staff). (5) Recently, Massachusetts hospitals collaborated on a voluntary patient safety initiative and developed practice recommendations that emphasize accurate and timely communication of critical test results. (6) In addition to timely reporting, the JC and CAP require that when critical results are communicated verbally, they be written down and read back to ensure that information has been transmitted accurately.
